👉 If you’re over 18 and need help navigating the health and social care system, our Care Coordinators can work with you to create a personalised care and support plan focused on what matters most to you. Whether you’re managing long-term conditions such as diabetes, asthma, COPD, dementia, or chronic pain, or if you're experiencing isolation or loneliness, our Care Coordinators will take the time to understand your personal situation and connect you to the right services and support.
👉 They work closely with you and your caregivers to ensure that you receive the best care possible and help you access local resources. From cancer support to long-term condition management, physical health assistance, and signposting to other services, our Care Coordinators are here to guide and support you every step of the way.
👉 Once referred, your Care Coordinator will get in touch to discuss how they can help and start developing a Care and Support Plan with you. They will continue to support you over several months, helping you take the steps necessary to improve your health and wellbeing.
